Wierd Morning results????
Question:
I am type II taking 50 units morning and 42 evening 70/30. I have been on this regimen since December with my doc adjusting my meds about every six weeks. I get morning BG’s around 170 and it doesn’t seem to matter what I eat at night. In fact I missed taking the insulin and the BG was still around the same. I have tested my meter and my BG does change during the day. I am also taking Metformin and Actos. Is this just my body adjusting to the changes in medication or what? Harold
The most powerful method of controlling morning bG is a dose of long lasting insulin at bedtime. However, you are taking a mixed dose of fast and slow insulins (70% slow, 30% fast). That makes one very suspicious of a Somoygi effect in which: a. the fast insulin sends you low in the middle of the night b. you rebound high c. the slow insulin spends the rest of the night slowly bringing you down It would be very useful to know what your 3 am sugars are like and then. It’s even more important for folks taking evening 70/30 If you are low at 3 am, that’s a signal to either cut back on the 70/30, or try straight NPH. More bad news. 70/30 is only suitable for folks making a lot of self-generated insulin. If you are taking metformin and Actos, and shooting those high doses of 70/30, you aren’t making much of your own insulin any more. That means your next step is MDI, Multiple Daily Injections of a fast and slow insulin in the same fashion as the Type 1 diabetics. MDI means more shots (ugh) but will give you much better control. . .perhaps even much, much better control. Take a look at the Brady J. King "Good and not so good news" thread on alt.support.diabetes. He hit HbA1c of 5.1 as a T2 on MDI with the most modern and powerful insulins, Humalog and Lantus. If T2 are willing to go to the bother of MDI, they often can achieve spectacular control. Regards Old Al
